Registration for Annual Student State Conference

All schools planning on attending the MASC/MAHS Annual Student State Conference must complete and submit this registration form by MARCH 13, 2009. Any submissions AFTER March 13th are subject to space availability.

REVIEW REGISTRATION POLICIES REGARDING LIMITATIONS OF STUDENTS PER ADVISERS AND CANCELLATION POLICY BEFORE CONTINUING. 

BEFORE STARTING, please make sure to have the following information on EVERY person you are registering, students and adults:

  • First Name
  • Last Name
  • Email Address
  • Home Mailing Address
  • Home Phone
  • Shirt Size
  • Grade Level (title/position for advisers/adults)
  • Dietary Restrictions (ex: vegetarian)
  • Special Needs
