Board of Delegates Application Form

Note: We are now accepting applications for the 1 year term in all centers with vacant seats.The Michigan Association of Student Councils and Honor Societies truly strives to be student-led in all of its programs and events. One of the greatest ways for secondary school students to grow in their walks of leadership--while helping to grow MASC/MAHS programs is to serve on the MASC/MAHS Board of Delegates. This team of students will meet 3-4 times per year in order to provide student guidance for the year's programs, learn facilitation skills in order to run MASC/MAHS events, and to provide a network of student leaders for your peers to connect with. Before beginning make sure to read over the 2011 Selection Procedures, as well as the MASC/MAHS Board of Delegates Expectations and Responsibilities. These are vital to your success as a prospective Board of Delegates (BOD) member, and you will be required to agree to them at the end of your application.Please read all of the details below regarding this application form.

Please make sure to complete the application in full once you begin.Once you have submitted the online application, you will automatically be given directions regarding the final certification process. You will be required to print and bring with you a form of certification/approval that will need to be signed by your MASC/MAHS adviser, buidling principal, parent/guardian, and YOU the candidate. This form will need to be received to MASC/MAHS should you be selected for one of the open seats..

This form must be completed no later than 11:59pm March 18, 2011! There will be no opportunities for late applicants.

Primary communication about the selection process will be in email. Please list an address that you check daily (or more frequenly!).

Please use your full mailing address for record purposes.

*These levels will be considered for an at-large Middle Level delegate position.

Please select any of the following activities in which you are a member IN GOOD STANDING. You must be a member of one of these activities in order to be considered for the MASC/MAHS Board of Delegates.

Please provide MASC/MAHS with the phone number that is EASIEST to reach you after-school. This can be a home phone or a cell phone.

Please provide MASC/MAHS with parent and/or guardian information. They MUST be informed in your interest with our organization as well as the responsibilities and procedures.

Please enter your birthdate in xx/xx/xxxx format.

The first file that you will be requred to submit is a resume of your co-curricular involvements. Please adhere to the following guidelines when submitting your document.
**Document MUST be in Word (.doc) or PDF (.pdf) format.
**Document should contain FOUR categories (listed as subheaders)
1.) Student Clubs and Activities (both school and non-school related)
2.) Volunteer Service
3.) Conferences and Workshops (include ALL MASC/MAHS events)
4.) Awards and Honors
DO NOT insert any other information into this document--it will not be scored and may result in non-consideration of your application.

To be eligible for the Board of Delegates your MASC/MAHS member adviser must write a letter of recommendation. Please adhere to the following requirements for this submission:
**File must be in Word (.doc) or PDF (.pdf) format.
**Your letter should include length of time that the adviser has known you, exceptional qualities that they notice in you, and any other information that would be helpful to the selection committee.
**Please place on school/club letterhead if at all possible.
**Adviser must be from either your student government, honor society, class council, or leadership class involvement.

The final submission that you are required to make is an essay addressing the following thoughts:
-Why do you want to be a part of the MASC/MAHS Board of Delegates?
-Why do you think that YOU would be a good fit on the Board? What specific qualities, talents, or attributes will you bring?
-Why do you think it is important to be involved in your school?

Your submission should be no longer than 1200 words (double check in a word processing program!) and should be COPIED and PASTED as plaintext into this box.

DO NOT include your name, or your school name in the essay (get creative!) as they will be evaluated anonymously.

Please submit an electronic photo of yourself. This will be referenced once applications have been graded. This photo should be a headshot (not full body or group shot) and should be smaller than 1 MB.

Please check each of the boxes when you have read AND understood the concepts listed. This is a required part of the application processed, and those who do not comply will not be able to proceed in the selection process.
